This information is a requirement to receive my services and by purchasing a service, the client chooses to provide such information. This data, whether given orally, in writing or electronically will be held with the utmost confidentiality in a secure confidential digital file for a period of seven (7) years from the last information provided, unless the client instructs otherwise.
A client may withdraw consent to the use of personal information at any time. I will respect your decision, but I may not be able to provide you with certain services if I do not have the necessary personal information.
How do I use and Disclose Personal Information?
I use client personal information you provide only for the purpose for which the information was collected. For example, I require a client’s personal information to stay in contact with the client for consultation purposes, for billing purposes and also to gather health history data so I can better understand my client’s health concerns to which they are seeking my guidance.
How do I Safeguard Personal Information?
I make every reasonable effort to ensure that personal information is accurate and complete. If you are aware of an error in my information about you, please let me know and I will correct it on request whenever possible. I make every reasonable effort to prevent any loss, misuse, disclosure or modification of personal information, as well as any unauthorized access to personal information. I use appropriate security measures when destroying personal information, including shredding paper records and permanently deleting electronic records. I also use a password protected device.
Access to Records Containing Personal Information
Individuals have a right to access their own personal information that is in the safeguard of Michelle Haley DHHP.
